We pair this with a 2-week continuous glucose monitor to understand how your blood sugar patterns interact with your symptoms. From there, your physician develops a personalized restoration plan that may include targeted nutrition, supplementation, lifestyle optimization, and prescription medication when clinically appropriate — all tailored to your specific PCOS phenotype and health goals.

The PCOS Foundations Program includes:

  • Comprehensive 45-minute initial physician consultation including symptom review, focused assessment, program orientation, and onboarding

  • Advanced laboratory testing to assess key markers related to PCOS, including insulin resistance, metabolic health, androgen balance, inflammation, and overall hormone function — with detailed physician interpretation

  • 2-week continuous glucose monitor (CGM) to provide real-time insight into blood sugar patterns and metabolic health

  • Targeted supplement recommendations tailored to your unique physiology, symptoms, and health goals

  • Prescription medication when clinically appropriate to support hormone balance, metabolic function, and symptom improvement (cost of medication not included)

  • A personalized plan with step-by-step guidance you can implement at your own pace

  • Educational resources and supportive tools throughout the program

  • 12 weeks of messenger support through the HIPAA-compliant patient portal

  • Discounted follow-up visits you can book as needed for ongoing physician guidance and individualized support throughout your healing journey

Frequently Asked Questions

Are health optimization programs covered by insurance?

We do not contract with insurance companies for our clinical programs.  We intentionally chose this model of care for our practice to prevent delays in receiving care (due to waiting for insurance approval) and third parties making decisions about your health.  This enables us to provide you with the flexibility, autonomy, and unhurried care we believe is the best approach to medicine.  Click here to learn more.

However, depending on your out-of-network benefits, you may be able to get some or all of the visit fees reimbursed.  We can provide a superbill (a detailed receipt) that you can submit to your insurance for possible reimbursement.  We do not submit claims or contact insurance companies on your behalf.

What are the fees associated with the programs?

The program is $1497 or 3 payments of $499 for 12 weeks of physician-guided support.

What happens after I complete the initial 3-month program?

In our experience, treating hormonal imbalance is not a quick-fix.  It requires time, consistency, and patience.  Because we are committed to your health, all of our clinical programs require an initial 3-month commitment to give you the time and space necessary to heal.

At the end of the initial 3-month program, we have a much clearer sense of what’s working well and what may still need adjustment.

At that point, you have the option to:

1. Discontinue care if your treatment goals have been met.

2. Continue active treatment for another 3-month period if new goals arise or more refinement is needed.

3.  Transition to a maintenance plan at a lower cost to maintain your progress. This includes ongoing prescription medications, quarterly telehealth visits, ongoing provider support.

Where do you see patients?

We provide telehealth services to women over 21 years old who are in the states of: NY, PA, MD and FL at the time of their visit.

Are labs and supplements included in your programs?

Initial advanced stool testing is included in the cost of the program. Follow-up testing can be done as needed for an additional fee.

Supplements are available at a 10% discount from our online dispensary.
